Established is the MCP floor — the level a managed care plan contract requires.
The Structural Reality:
MCP contracts require Established across all four layers at once. Most community-based providers sit between Developing and Established. That gap is not failure. It is underfunded potential, and naming where you stand is the first step to closing it.
